To get the most out of your appointment, please bring copies of any blood tests or referral information from your doctor and a list of any medications or supplements you are taking. You may also need your medicare or private health card.

It can be helpful to complete a food diary in the days or weeks leading up to your appointment.

To save you time on the day of your appointment, you can download your new appointment forms and a template for your food diary from this page. Alternatively, you can arrive at reception 5 minutes early to complete your forms.
